- Occupational Therapist (O.T.)
-
Responsible to provide therapy services to patients having significant problems requiring the skills of the therapist to enable patient to become more capable of activities of daily living.
FUNCTIONS:
1. Completes evaluations on patients referred for occupational therapy to include physical, neurological, muscular, skeletal, cognitive and psychosocial areas and how they impact the clients level of independence and safety.
2. Completes individualized treatment plans utilizing evaluation data and in conjunction with physicians orders, the client, the clients family when appropriate, and other team members.
3. Completes documentation in a timely and accurate manner according to Medicare guidelines and agency guidelines.
4. Provides treatment in the most cost effective manner and with the best quality possible, utilizing a variety of treatment modalities, techniques and equipment.
5. Monitors the clients progress and modifies treatment accordingly.
6. Functions as an integral part of the treatment team.
7. Establishes and maintains effective relationships with community resources for adaptive equipment, services and consultation.
8. Identifies and pursues professional goals to upgrade and maintain the highest professional standards for quality client care.
9. Participates in consultation/supervision on a regular basis and seeks the same as needed.
10. Complies with all company policies and procedures.
11. Participates in agency and company meetings as scheduled/requested.
QUALIFICATIONS:
1. Must hold a minimum of a BS or BA from an accredited occupational therapist educational program.
2. Must successfully have completed the therapist level field work requirements as stipulated in the Essentials of an Accredited Educational Program for Occupational Therapist.
3. Must have successfully completed the Certification examination for Occupational Therapist, Registered.
4. Will have paid the AOTA certification and registration fee for Occupational Therapist, Registered.

Assesses patient care and nursing needs and takes appropriate steps to provide high quality nursing care services to patients of the agency.
FUNCTIONS:
1. At onset of assignment, identifies and documents patients condition and establishes nursing care plan.
2. Re-evaluates patients nursing needs on each patient contact.
3. Initiates the nursing care plan of treatment and makes necessary revisions.
4. Provides those services requiring substantial specialized nursing skills.
5. Initiates appropriate preventative and rehabilitative nursing procedures.
6. Prepares clinical and progress notes.
7. Informs the physician, families, co-directors and other personnel of changes in the patients condition and needs.
8. Counsels the patient and family in the patients care needs in the intervals between agency visits.
9. Participates in teaching families and other personnel about patient care techniques.
10. Identifies need for professional knowledge and obtains continuing education that will upgrade and improve ability to provide high quality care.
QUALIFICATIONS:
1. Graduate of an approved school of nursing.
2. Licensed to practice nursing in the State of Colorado.
3. Minimum of one year of experience in a supervised patient care setting providing medical/surgical/pediatric care.
4. Preference given to experience in the home health care field or pediatric care field.

QUALIFICATIONS:
1. Must hold a minimum of a Masters degree in the field of Speech Therapy from an accredited educational program.
2. Meets the education and experience requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech-Language-Hearing Association; or meets the educational requirements for certification and is in the process of accumulating the supervised experience required for certification.

- Physical Therapist (PT)

Responsible to provide therapy services to patients having significant problems requiring the skills of the therapist to enable patient to become more capable of activities of daily living.
FUNCTIONS:
1. Assists the physician in evaluating the patients level of function by applying muscle, nerve, joint and functional ability tests.
2. Participates in developing the plan of treatment to improve the patients level of functioning.
3. Provides treatment modalities consistent with the plan of treatment to relieve pain, develop or restore function, using physical means, such as exercise, massage, heat, water, light and electricity.
4. Directs and aids patients in active and passive exercises, muscle re-education and gait training, activities of daily living including transfer techniques and prosthetic training.
5. Uses equipment such as ultraviolet and infrared lamps, low voltage generators, diathermy and ultrasonic machines.
6. Evaluates ongoing progress in patients condition in relation to care given and revises plan of care appropriately.
7. Teaches patient/family and other care givers, exercise routines, use of wheelchair, braces, crutches, canes and prosthetic and orthotic devices.
8. Prepares clinical notes and progress notes according to agency policy.
9. Participates in case conferences and other agency evaluation activities as requested, and agency quarterly chart review.
QUALIFICATIONS:
1. Meets the qualifications as established by APTA or other professional organization.
2. Licensed to practice physical therapy in the state of Colorado.
3. Experienced in pediatric rehabilitative services.

Provides routine nursing care of the patient within the confines of licensure limits.
FUNCTIONS:
1.Gives and demonstrates nursing care to patients and families.
2. Follows agency policies and performs nursing procedures within scope of legally authorized practice.
3. Observes and records patients condition or situation, notifies R.N. of need for change in care plan.
4. Reports immediately, changes in patients condition to Director of Patient Care Services and follows instructions as given.
5. Assists in developing and carrying out nursing care plans.
6. Communicates with the physician changes in patients condition, response to medication and other appropriate information according to agency policies.
7. Identifies need for CEUs and participates in educational opportunities as needed.
QUALIFICATIONS:
1.Graduate of an approved school of practical nursing.
2. Licensed to practice practical nursing in the state of Colorado.
3. Minimum of three years experience in a supervised patient care setting providing medical/surgical nursing care.
4. Preference given to prior experience in the home health care field.
LIMITATIONS:
1. Can not do the initial assessment of patients.
2. Can not do the patient plan of care.
