Assesses patient care and nursing needs and takes appropriate
steps to provide high quality nursing care services to patients of the agency.
FUNCTIONS:
1. At onset of assignment, identifies and documents patients condition and
establishes nursing care plan.
2. Re-evaluates patients nursing needs on each patient contact.
3. Initiates the nursing care plan of treatment and makes necessary revisions.
4. Provides those services requiring substantial specialized nursing skills.
5. Initiates appropriate preventative and rehabilitative nursing procedures.
6. Prepares clinical and progress notes.
7. Informs the physician, families, co-directors and other personnel of changes
in the patients condition and needs.
8. Counsels the patient and family in the patients care needs in the intervals between agency visits.
9. Participates in teaching families and other personnel about patient care techniques.
10. Identifies need for professional knowledge and obtains continuing education that will
upgrade and improve ability to provide high quality care.
QUALIFICATIONS:
1. Graduate of an approved school of nursing.
2. Licensed to practice nursing in the State of Colorado.
3. Minimum of one year of experience in a supervised patient care setting
providing medical/surgical/pediatric care.
4. Preference given to experience in the home health care field or pediatric care field.
